As a Registered Nurse Hospice Case Manager at Spero Hospice, you'll be at the heart of compassionate, patient-centered care. $53 - $56 hourly

Responsibilities:

• Develop and implement individualized care plans to meet the unique needs of each patient.
• Coordinate with physicians, social workers, and other healthcare professionals to ensure seamless care delivery.
• Provide compassionate support and education to patients and their families, helping them navigate the hospice journey.
• Conduct regular assessments to monitor patient conditions and adjust care plans as needed.
• Administer medications and treatments as prescribed, ensuring adherence to best practices and safety protocols.
• Document patient progress and maintain accurate records to support effective communication within the care team.
• Advocate for patients' needs and preferences, ensuring their voices are heard and respected in care decisions.

Qualifications:

• Current Registered Nurse (RN) license in the state of New Jersey.
• Experience in hospice or palliative care, demonstrating a deep understanding of end-of-life care.
• Ability to develop and implement individualized care plans tailored to each patient's unique needs.
• Proven track record of effective collaboration with interdisciplinary teams, including physicians and social workers.
• Strong communication skills to provide compassionate support and education to patients and families.
• Proficiency in conducting regular assessments and adjusting care plans based on patient conditions.
• Commitment to advocating for patients' needs and ensuring their preferences are respected in care decisions.
