About the Role

The Social Services Director is responsible for supporting the psychosocial well-being of residents and their families while ensuring compliance with applicable state and federal regulations and facility policies. This role works closely with residents, families, nursing, physicians, mental health professionals, and other members of the interdisciplinary team to ensure each resident's social and emotional needs are identified and addressed.

Key Responsibilities

• Complete the Social Services portion of the MDS, Resident Assessment Protocols, care plans, progress notes, and social services assessments accurately and within required timeframes.

• Provide support to residents and families regarding social, emotional, adjustment, and family-related concerns.

• Participate in interdisciplinary care planning and identify residents' psychosocial and emotional needs.

• Maintain timely and accurate documentation of resident progress, interventions, and response to the plan of care.

• Make appropriate referrals to community resources and support agencies and maintain documentation of referrals.

• Participate in discharge planning and help residents and families prepare for a safe and successful transition.

• Serve as a resource for residents, families, legal representatives, and the community regarding facility services and resident needs.

• Visit residents regularly and provide support, including facilitating communication with family members and connecting residents with appropriate community resources.

• Educate residents and/or their legal representatives regarding Resident Rights and advocate for resident needs.

• Coordinate logistics and communication for monthly Family Council Meetings .

• Coordinate resident room changes as needed.

• Contact residents and/or legal representatives to encourage participation in care plan meetings and resident care discussions.

• Maintain appropriate communication with residents, legal representatives, and approved family members regarding resident needs and status.

• Collaborate with the Activities Department to help meet the needs of residents who have limited or no family support.

• Work closely with the facility's mental health provider to identify psychosocial needs, coordinate referrals, and ensure appropriate services are provided.

• Promote positive relationships among residents, families, administration, medical staff, physicians, consultants, employees, volunteers, community agencies, and other facility partners.

• Assist with community education regarding aging, Resident Rights, facility services, and other relevant topics.

• Orient new residents to the long-term care environment and assist with their adjustment to the facility.

• Perform other duties as assigned by facility leadership.

Qualifications

• Bachelor's degree in Social Work or a bachelor's degree in a related human services field, including but not limited to sociology, special education, rehabilitation counseling, or psychology.

• Minimum of one year of supervised social work experience in a healthcare setting working directly with individuals.

• Previous social work experience in a skilled nursing, long-term care, rehabilitation, or healthcare setting preferred.

• Strong verbal and written communication skills.

• Excellent organizational skills and attention to detail.

• Ability to maintain confidentiality and exercise sound professional judgment.

• Strong interpersonal and customer service skills.

• Ability to communicate effectively with individuals at all levels of the organization.

• Ability to work collaboratively as part of an interdisciplinary team.

• Ability to manage multiple priorities and work effectively in a fast-paced environment.
