Job Description
Join Our Vibrant Team as a Case Manager!

Job Description:
- Start Date: ASAP
- Shift: Day 5x8-Hour (08:00 - 16:30)
- Patient Ratios: 1:35
- Years of Experience Required: 2 years of acute hospital case management
- Weekend Requirement: Rotate every other weekend and holiday coverage
- Location: California (CA state license preferred; pending license accepted)

Key Responsibilities:
- Oversee up to 2 units on weekends.
- Focus on effective discharge planning.
- Collaborate with a multidisciplinary team to ensure continuity of care.
- Conduct comprehensive biopyschosocial assessments.
- Document care coordination, including progress notes and treatment plans.
- Educate patients and families to facilitate successful discharges.
- Participate in family meetings and treatment plan development.
- Navigate various health and social services, including DCF and financial assistance programs.

Patient Demographics:
- Neonates, Infants, Pediatrics, Adults, Geriatrics
- Daily census of up to 25 patients

Qualifications:
- 2 years of acute hospital case management experience required.
- Proficiency in Epic charting system.
- Experience with high-risk patient populations, including mental health and substance abuse.
- Strong communication and organizational skills are essential.
- Ability to work collaboratively in a fast-paced environment.

Additional Information:
- Computerized documentation skills are required.
- Must be flexible for assignment changes and floating to nearby facilities.
- RTO restrictions: No more than 7 days off requested upfront.
