Advanced Practice Clinician (FNP)-Acute Care Transition Team

Position Summary As a key member of the Acute Care Transition Team (ACTT), the ACTT Advanced Practice Clinician will contact patients within 48 hours of discharge from the acute care setting to achieve a 100% contact rate. They will answer general and specific questions regarding the patient’s medical encounter, plan of care, potential complications, and discharge instructions. The clinician will work with team members to bridge the divide between outpatient and hospital care, supporting patients, their families, and the acute care staff as they navigate the healthcare system at White Plains Hospital (WPH) and its affiliates. The clinician will facilitate two‑way communication between the acute clinical setting and the home environment for encounters from the emergency department, inpatient setting, observation, labor & delivery, and ambulatory surgery. Essential Functions and Responsibilities Understands and adheres to the WPH Performance Standards, Policies and Behaviors. The ACTT Advanced Practice Clinician will contact patients within 48 hours of discharge from the acute care setting with a goal of 100% contact rate. They will answer general and specific questions about the patient’s medical encounter, plan of care, potential complications, and discharge instructions, focusing on diagnosis management, medication use, and social determinants of health that may impede recovery. They will serve the following patient populations: High‑risk patients in transition home Patients with medication or pharmacy issues Patients requiring radiology or microbiology follow‑up Incoming acute‑care transfers from outside organizations or affiliates Patients and families with clinical concerns or questions Patients referred by a member of the ACTT Provides medical care and advice under the supervision of an assigned physician, documenting all interactions in the medical record for future review by treating providers and for QA/PI purposes. Assesses the patient experience, collects feedback, and provides service recovery when appropriate. Assists with the collection and dissemination of data, utilizing appropriate software and documenting in the medical record. When another ACTT member can assist, the contact is transferred to that co‑located team member (e.g., ACTT RN, Social Worker, Physician Referral Navigator Specialist). Performs all other related duties as assigned. Education and Experience Requirements Current New York State registration as a Physician Assistant or Family Nurse Practitioner. Current White Plains Hospital Center Collaborating Physician Certification Form Agreement. Five years of experience as a nurse in an appropriate clinical setting with inpatient, emergency department, or care coordination experience preferred. Retains composure under stress. Patient engagement training preferred. Strong computer skills, including Outlook, Word, Excel, PowerPoint, EPIC, MediTech, and Midas. Can type at 50 words per minute. Heavy phone use. Requires extensive computer use. Must respond to frequent calls. Remains in a stationary position approximately 90% of the time. May need to move within the office and hospital. Constant computer use to access and document information. Communicates with patients, families, and hospital employees via telephone and email, exchanging accurate information.
