Key Responsibilities

• Conduct comprehensive assessments of residents to diagnose both acute and chronic medical conditions accurately.
• Collaborate with physicians and nursing staff to formulate and implement evidence-based treatment plans tailored to individual resident needs.
• Order, interpret, and follow up on diagnostic tests as necessary, while prescribing medications and therapies in line with best practices.
• Educate residents and their families on effective disease management, medication adherence, and transitional care strategies post-discharge.
• Engage actively in interdisciplinary rounds, providing insights and updates on resident progress while contributing to coordinated care efforts.
• Maintain meticulous electronic medical records, ensuring compliance with regulatory standards and enhancing continuity of care.
• Participate in quality improvement initiatives aimed at elevating the standard of care and implementing evidence-based practices across the facility.
• Stay abreast of advancements in geriatric care and nursing practices to continually enhance service delivery.

Patient Population / Client Focus

This role focuses primarily on seniors living in our Assisted Living Facility, many of whom may face complex medical and social challenges. You will be instrumental in bridging the gap between acute medical needs and long-term care, providing compassionate, personalized health services that empower residents to maintain their independence and dignity.

Qualifications

• Current certification as a Nurse Practitioner.
• Active and unrestricted nursing license in the state of Florida.
• Bachelor's Degree in Nursing from an accredited program.
• A minimum of four years of clinical experience as a Nurse Practitioner, preferably in a geriatric or assisted living setting.
• Strong communication and interpersonal skills, allowing you to effectively build rapport with residents, families, and the healthcare team.
• Demonstrated critical thinking, organizational skills, and the ability to manage multiple priorities in a fast-paced environment.

Preferred Experience

Experience working in assisted living or similar environments is highly desirable. Familiarity with electronic health record (EHR) systems and best practices in geriatric care will be an advantage, as will a commitment to ongoing professional development and learning.

Compensation and Benefits

The compensation for this full-time position ranges from $120,000 to $220,000 annually, commensurate with experience and qualifications.
