Hope the Mission in Sun Valley, CA seeks a Housing-Focused Case Manager to deliver individualized, housing-first case management for interim housing participants, prioritizing rapid permanent housing placement using low-barrier and trauma-informed approaches. Under supervision of the Program Manager, you will assess housing barriers, develop Housing Action Plans, maintain regular contact, and coordinate with housing partners to ensure successful tenancy and stability. #J-18808-Ljbffr Hope the Mission
