As a PMHNP on our Intensive Case Management Team, you’ll care for individuals experiencing serious mental illness, substance use disorders, homelessness, and complex medical needs within a multidisciplinary model that includes therapists, case managers, primary care providers, addiction specialists, and peers. What You’ll Do Provide psychiatric evaluations, diagnosis, medication management, and ongoing treatment. Prescribe buprenorphine, Vivitrol, Naltrexone, Acamprosate, and other recovery‑supporting medications. Deliver brief psychotherapy and motivational‑interviewing interventions. Respond to psychiatric crises with suicide risk assessment and intervention. Collaborate closely with behavioral health, primary care, and substance‑use treatment teams to craft individualized treatment plans. Why Join Central City Concern? Minimum Qualifications Bachelor’s degree in nursing and a master’s degree in Psychiatric/Mental Health Nursing from an approved school. Current Oregon nursing license and certified PMHNP by the Oregon State Board of Nursing and national certification. Full prescriptive powers in Oregon with a DEA number. Valid BLS certification and buprenorphine prescriber training. Ability to meet physical demands, walk, bend, lift, and perform tasks required for patient care. Successful completion of pre‑employment drug screen, TB test, and background check (including DHS clearance). Adherence to Central City Concern’s drug‑free workplace policy and confidentiality regulations (HIPAA, 42 CFR Part 2). Current driver’s license and compliance with the Fleet Safety Policy if a vehicle is required. Essential Duties and Responsibilities Deliver comprehensive, client‑centered, trauma‑informed psychiatric services within the multi‑disciplinary team. Identify opportunities to improve service integration and workflow efficiency while following information‑sharing guidelines. Maintain accurate, program‑standard client records and participate in quality‑assurance activities. Provide crisis intervention, suicide assessment, and emergency care coordination. Offer brief psychotherapy, motivational interviewing, and patient/family education. Advance skills through training and seek consultation when needed. Ensure compliance with privacy, confidentiality, and security policies. Perform additional duties as assigned by supervisory staff. Skills & Abilities Excellent functional assessment and therapeutic decision‑making within PMHNP scope. Expertise in pharmacological and psychotherapeutic interventions for substance use disorders and psychiatric conditions. Ability to build rapport rapidly and provide non‑judgmental, compassionate care to vulnerable populations. Strong documentation skills and adherence to regulatory requirements. Interpersonal skills for working collaboratively with diverse teams and clients. Physical capability to perform tasks associated with clinical duties.
