Licensed Vocational Nurse (LVN) – Home Health

???? Location: San Luis Obispo
???? Pay: $60–$70 per visit

Guardian Angel Home Care is seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to join our growing home health team. In this role, you will provide high-quality, patient-centered care in the comfort of patients’ homes while working alongside a supportive interdisciplinary team.

• Provide skilled nursing care in patients’ homes

• Monitor patient condition, vital signs, and overall health status

• Administer medications and treatments as ordered by the physician

• Accurately document all visits and care provided

• Communicate changes in patient condition to the RN/physician

• Educate patients and families on care plans, medications, and disease management

• Ensure compliance with all state, federal, and agency regulations

• Collaborate with the interdisciplinary team to ensure optimal patient outcomes

• Active California LVN license in good standing

• Graduate of an accredited practical nursing program

• BLS certification

• Strong clinical, communication, and documentation skills

• Ability to work independently in a home health setting

• Home health or hospice experience

• Experience with Homecare Homebase (HCHB) or other EMR systems

• IV therapy, wound care, or other specialty certifications

• Bilingual is a plus
