Licensed Practical Nurse - Home HealthSchedule: 3x12sThe Licensed Practical Nurse (LPN) provides direct participant care, supports providers during examinations and procedures, and assists with care coordination in a clinic setting. This role ensures efficient patient flow, delivers high-quality nursing care within scope of practice, and supports clinical operations to promote positive participant outcomes.PACE or Program of All-inclusive Care for the Elderly is a federal and state funded program that provides comprehensive, coordinated, and proactive whole-person care to help maintain and improve seniors' health so they can live safely in their homes.
