Registered Nurse (RN) - On Call - Trinity Health at Home - Grand Haven, Full Time
To provide nursing assessment, planning and care to maximize the comfort and health of patients and families in accordance with the interdisciplinary plan of care after regular business hours and/or weekends. Must communicate well with patient, caregivers, and other health care professional team members while maintaining the standards of professional nursing. Positive attitude and team work are essential.
In this role, you will have the ability to:
- Manage the overall care and well-being of patients and their families at end-of-life
- Collaborate with an interdisciplinary team to provide holistic care to our patients
- Access our Health system as a great support network with immediate access to patient care resources
- Serve a smaller, intimate patient base
- Operate within a radius of services within 30 miles of Grand Haven
What are the Minimum Skills, Experience and Educational Requirements?
- Valid license as a Registered Nurse as issues by the State of Michigan.
- A minimum of 2-3 years experience as a RN, with some Hospice or Home care nursing preferred.
- Excellent assessment and critical thinking ability.
- Current CPR certification.
- Must be able to practice independently.
- Working knowledge of adult disease processes.
- Must have valid drivers license, current auto insurance and a reliable automobile.
- Comfortable with computer charting.
- Able to read, write legibly and speak the English language effectively.
- Must be self directed with the ability to work with minimal supervision.
- Must be available to work weekday nights and weekends.
What are the Essential Job Functions and Responsibilities?
- Clinical
- Utilizes the nursing process of assessment, planning, interventions, implementation, and evaluation to provide care to hospice patients which will maximize comfort at end-of-life.
- Performs scheduled (as directed) and non-scheduled home visits/phone calls to initiate or continue care after hours. Responds to client care issues and communicates with RN Case Manager and IDT with assessment and plan of care changes and on going needs.
- Responds promptly to calls from answering service. Communicates effectively with patients, caregivers, referral sources and facilities.
- Triaging patient related calls by determining the needs of the caller and then responding in an appropriate manner either by phone or with a visit. Consults with physician on-call as is needed.
- Educates patients/families about the disease process and appropriate care techniques.
- Respond to referral calls, providing the caller with information regarding hospice care, screening calls for appropriateness, taking patient information and coordinating with the Intake nurses for admission.
- Performing patient information visits and admission visits as assigned by Intake or supervisor.
- Performing other duties within scope of practice and as training permits.
- Reports concerns to the hospice supervisor on-call.
- Documenting and communicating all calls, interventions, care plan changes, physician contacts in a timely manner consistent with policy and procedure.
- Performing as a core member of the the Hospice of North Ottawa Community interdisciplinary team and participates in the Total Quality Management philosophy of the agency.

- Orients/educates staff/volunteers to the role of the nurse and the care of the dying patient.
- When on call, initiates communication with attending physicians, other hospice staff members, and other agencies as needed to coordinate optimal care and use of resources for patients/families served while on call

- Participates in peer review and team evaluation assuring high quality care is being provided.
- Strives for continual improvement in systems and processes.
- Professional Development
- Attends educational in-services.
- Will obtain or be working towards becoming certified in Hospice and Palliative nursing.

- Renders care in patient home setting exhibiting a diverse social, cultural and economic ranges. May encounter hostile pets and difficult access to homes.

- Non-traditional work hours (on call, weekends and holidays).
