Medical CoordinatorAdvanceCare Health Services, LLC is seeking a dedicated Medical Coordinator with DDA/ECF/CLS experience to help us with our Medical Department. The ideal candidate will be responsible for managing administrative tasks and assisting in the smooth operation of the department. Working at home up to 4 days a week after initial training.This is a hybrid role, combining work-from-home flexibility with some on-site and delivery responsibilities once weekly.You will need to have a 27" or larger monitor, keyboard, and mouse to use with our company-issued laptop. (If you are coming to the office to pick up your equipment, we can loan you the monitors, keyboard, and mouse as well).Applicants must have high-speed internet.Duties:Function as a medical case manager for the assigned individuals served in our Family Model ProgramMonitor the individual's service plan in accordance with the regulatory guidelinesCoordinate and arrange appointments, meetings, and other administrative matters on behalf of individuals served or staff membersAssist in a supportive role for the individual served and his/her familyWork with other Agencies on behalf of the assigned individual servedAssist Director in interviewing prospective individuals served and their familiesUtilize computerized systems to schedule appointments and maintain patient recordsPerform administrative duties such as filing, data entry, and office managementUtilize medical terminology accurately in communication and documentationWork with the Director in promoting community acceptance and support through participation in community activities and/or liaison efforts with other agencies and/or entitiesAssist in the performance of other duties as required by the needs of individuals served and as assigned by the DirectorPerform other duties and responsibilities as assigned and as neededQualifications:Cooperates with anyone who evaluates the home or represents the various agencies that place individuals in the homeAbility to prepare necessary reports and records of individuals' program progress with neatness, accuracy, and completenessKnowledge of practices and techniques of programming for individuals with intellectual disabilities Ability to exercise judgment in interpreting and implementing programsAbility to implement proper practices and techniques of supervisionAbility to maintain effective verbal and written communication with staff, individuals' families, individuals and outside service providersMust be flexible and organizedStrong administrative skills with attention to detailAbility to multitask and prioritize tasks effectivelyMust maintain liability insurance, good driving record, and reliable transportationMust be able to work without close supervisionThe ability to interact with a wide range of peopleGood Computer skills: MS Word, Excel knowledge and Excel Spreadsheets, Outlook, Google Docs, and Windows.Work Environment:This full-time, 40-hour-per-week role offers a hybrid work arrangement, using company-provided systems to stay connected and productive. It requires a quiet, professional workspace when working from home, reliable internet, and the ability to stay focused and organized without direct supervision.
