Case Manager II - Empowering Communities & Benefits Access

State of Delaware is seeking a Health/Human Service Case Manager II to determine eligibility for needs-based benefits (SNAP, Medical Assistance, CHIP, GA, TANF) and coordinate with TANF/SNAP vendors. You will perform case management, provide referrals, and connect clients with housing, employment, and food resources. As part of a multidisciplinary team, you will use trauma-informed approaches to identify barriers, document cases accurately, and communicate with diverse populations while ensuring #J-18808-Ljbffr State-of-Delawar
