Job Details Job Location: Menders by Lenbrook, LLC - Atlanta, GA 30319 Job Purpose The Clinical Care Coordinator LPN at Menders by Lenbrook LLC is responsible for overseeing and coordinating clinical care activities to ensure high-quality patient outcomes. This role involves collaborating with healthcare professionals, managing patient care plans, and ensuring compliance with healthcare regulations and standards. Key Responsibilities Coordinate and manage patient care plans in collaboration with healthcare providers to ensure effective and efficient delivery of clinical services. Monitor patient progress and make necessary adjustments to care plans to optimize health outcomes. Serve as a liaison between patients, families, and healthcare providers to facilitate communication and ensure patient needs are met. Ensure compliance with healthcare regulations, policies, and procedures to maintain high standards of care. Provide education and support to patients and families regarding health conditions, treatments, and care plans. Assist in the development and implementation of clinical protocols and procedures to enhance service delivery. Participate in quality improvement initiatives to continuously improve patient care and service efficiency. Maintain accurate and up-to-date patient records and documentation in accordance with regulatory requirements. Collaborate with interdisciplinary teams to ensure comprehensive and coordinated care for patients. Qualifications Required Education Licensed Practical Nurse (LPN) certification from an accredited nursing program Required Experience Minimum of 2 years of clinical nursing experience, preferably in a care coordination role Experience working with geriatric populations or in a long-term care setting Familiarity with electronic health records (EHR) systems Required Skills and Abilities Strong organizational skills with the ability to manage multiple tasks and priorities effectively Excellent communication skills, both verbal and written, with the ability to interact professionally with patients, families, and healthcare teams Proficient in using computer applications and software related to healthcare management Ability to work collaboratively in a team environment and demonstrate leadership in coordinating patient care Knowledge of healthcare regulations and compliance standards relevant to patient care and coordination #J-18808-Ljbffr Menders
