Job Description Position Summary The Lead LPN is responsible for the proper and adequate day to day functioning of the clinic. This position ensures efficient patient flow, patient care, patient issues and also the supervision of the nursing staff, including staff performance and staff issues. $29.00 - $34.12 Maintains a professional environment in a multi provider, multidiscipline organization. Maintains a patient centered environment including but not limited to: Provision of care and advocate for patients of all ages and their families based on prudent nursing judgment and acceptable standards of practice. Efficient patient flow, assisting providers and other staff. Perform tasks and procedures within the scope of a Licensed Practical Nurse (L.P.N) and legal guidelines for the Nurse Practice Act. Perform nursing tasks in all clinical areas. Qualifications Graduate of an accredited school of nursing. Current state licensure to practice in Massachusetts. Cardio-pulmonary resuscitation (C.P.R.) certified. Maintains licensure and certifications. Bilingual preferred. Minimal Knowledge Base Demonstrates knowledge of: Nursing principles including disease process, certain examinations, observations, laboratory studies and appropriately apply these principles to patient care. Nurse Practice Act/Code of Ethics Nursing Process Managed Care OSHA Guidelines Infection Control Standards Emergency Procedures Moderate Computer Skills Reports To R.N. Supervisor and Clinical Director Principal Duties and Responsibilities Professional Behavior Maintains leadership and supervisory skills in a professional and objective manner. Demonstrates characteristics of accountability and responsibility. Is dependable and reliable as demonstrated by excellent attendance, punctuality and thorough follow through of work tasks. Maintains patient confidentiality at all times. Maintains discretion of conversation in work areas. Is pleasant, courteous, and considerate of patients and co-workers. Demonstrates ability to prioritize demands, work with distractions, adapt to change, exercise efficient time management, and work independently. Maintains appropriate chain of command. Attends 75% of nursing staff meetings, supervisor meetings and other health center meetings, and in-house in-services. Supervisory Responsibilities Daily tasks in the nursing department. Direct supervision of R.N.’s, L.P.N.’s, Medical Assistants, Phlebotomists, students and volunteers. Participates in orientation of new staff, students, and volunteers. Participation in nursing staff education and training. Participation in any nursing staff disciplinary action. Maintaining adequate staffing in conjunction with the budget. Assigning and re-assigns tasks to the nursing staff on a daily basis and as necessary. Is responsible for being on duty as the licensed coverage in the event that other licensed staff is not available. Maintaining a clean, neat, and ordeal medication room. Interacts with drug representatives. Oversees flow of patients into clinical area, re-assigning staff and rooms as needed. Completion of the staff’s annual evaluation. Oversees clinical tracking. Ordering medical supplies. Maintaining Medical Equipment. Tasks As Lead The liaison for the school system, department for children and families (DCF), The Department of Public Health (DPH), The Board of Health. Completion of payroll for the PEDI Nursing Staff. Assuring lunch breaks for the PEDI Nursing Staff. Vaccine manager for the health center Tracks 0-2 year old patients for monitoring of vaccine schedule. Patient Preparation for Visit Appropriately prepare patient for either a complete physical, sick visit, follow up visit; or a gynecological exam. Vital Signs: blood pressure, temperature, pulse (all point), orthostatic blood pressure and pulse. Obtain interpretation when needed. Chart documentation. Measurements: Height, Weight, and (other measurements as needed). Documentation of reason for visit, medication review, allergy review. Documentation of medication review. Patient Tests / Treatments Audio acuity testing Ear irrigation Electrocardiogram (EKG) Visual acuity testing Injections- Intramuscular (I.M.); subcutaneous (S.C.), Intradermal (I.D.); Z-track Medication administration Dose Calculations Peak flow meter Oxygen saturation Nebulizer treatments Oxygen administration Laboratory tests CLIA Waived Tests Pap Smear Handling Venipuncture- collect and process specimens Patient Procedures Physical and emotional assessment Wound care Assisting providers Maintaining sterile field and sterile technique Burn care Diabetic care Ace wraps/splints/slings/braces Biopsies Suture removal Staple removal Patient Centered Medical Home (PCMH) Participation in the PCMH includes but is not limited to: Overseeing completion of PCMH work by nursing staff assigned to their floor Delegating follow-up of patient hospital discharges to other nursing staff in the absence of the assigned nurse. Assists with hospital discharges as needed in order to maintain compliance with following-up on hospital discharges within forty-eight (48) business hours. Monitor reports for completion of care Other Patient Services Patient education Prescription refill Telephone triage Walk-in Triage Other related phone calls Referrals Appointments Nurse visits Documentation Interpreting as needed Care of Equipment Proper handling, cleaning, and storage of all equipment Other tasks as needed Quality Assurance Improvement Quality assurance monitoring- participates in the betterment of the health center through studies and reviews as necessary. Goal setting- assess personal, team, and organization goals. Participates in devising, reviewing, and revising policies, procedures and protocols. Assist with ongoing Quality Improvement activities throughout the health center. This includes, but it not limited to conducting Joint Commission tracers, serving on QI teams and committees as needed, assisting with staff education, and performing data reviews. Assist with ensuring staff compliance with HIPAA Privacy and Security Regulations on an ongoing basis through staff monitoring. Attends external conferences and meetings. Teamwork Orientation Maintains and encourages teamwork approach. Flexibility- continuously participates in the modification of schedule, duties, and procedural functions. Contribution- consistent effort to further goals by modifying and/or improving individual procedures and tasks. Manages conflict with staff appropriately Maintains and encourages good communication skills Participates in orientation and training of new staff and/or students. Working Conditions Works in a busy community health center in a downtown business district. Long hours are sometimes required. Physical Requirements Physical demands are requiring lifting of small pieces of equipment, boxes, and/or patients not to exceed forty (40) pounds. Mostly standing and walking required. Visual acuity is sufficient for frequent reading. Hearing acuity is sufficient for holding conversation with or without audio devices. This position will receive one-dollar ($1.00) per hour increase above their hourly rate.
