PLEASE NOTE: THIS IS PER-DIEM POSITION. PAID PER VISIT PLUS MILEAGE. Rate and mileage will be discussed during interview. We are looking for candidates in the following counties: Job Overview We are seeking a compassionate and dedicated Hospice/Health Aide to join our team. In this role, you will provide essential support to patients in a hospice setting, ensuring their comfort and dignity during end-of-life care. The ideal candidate will have experience in patient care and a strong commitment to enhancing the quality of life for individuals with terminal illnesses. Responsibilities Assisting patients with daily living activities such as bathing, grooming, and dressing. Monitoring and recording vital signs to ensure the well‑being of patients. Supporting patients with mobility needs, including lifting and transferring them safely. Documenting medical information accurately to maintain up‑to‑date patient records. Offering companionship and emotional support to patients and their families. Collaborating with healthcare professionals to implement individualized care plans. Requirements Previous experience in home care or hospice settings is preferred. CNA Certified HHA Certified Reliable transportation Knowledge of CPR techniques and ability to respond effectively in emergencies. Familiarity with medical documentation practices is an asset. Strong interpersonal skills with the ability to work compassionately with diverse populations. Ability to perform physical tasks including lifting and moving patients safely.
