Dietitian

Position Summary We are seeking a compassionate and knowledgeable Registered Dietitian to join our interdisciplinary team in a long-term care setting. The ideal candidate is committed to providing high-quality nutrition care that enhances residents' health, well-being, and quality of life while ensuring compliance with all federal, state, and local regulations. Key Responsibilities Complete comprehensive nutrition assessments for new admissions and at required intervals. Develop, implement, and monitor individualized nutrition care plans. Assess residents at nutritional risk and provide appropriate interventions. Monitor weight trends, laboratory values, hydration status, and nutritional outcomes. Collaborate with physicians, nursing staff, speech therapy, food service, and other interdisciplinary team members. Participate in care plan meetings and provide nutrition recommendations. Ensure compliance with CMS, state, and facility regulations regarding nutrition services. Review menus for nutritional adequacy and therapeutic diets. Provide nutrition education to residents, families, and staff. Maintain accurate and timely documentation in the electronic medical record. Participate in Quality Assurance and Performance Improvement (QAPI) initiatives. Requirements Qualifications Registered Dietitian (RD/RDN) credential through the Commission on Dietetic Registration (CDR). Bachelor's degree in Nutrition, Dietetics, or a related field (Master's preferred). Experience in long-term care, skilled nursing, rehabilitation, or geriatric nutrition preferred. Strong assessment, documentation, and communication skills. Ability to work independently and collaboratively within an interdisciplinary team. Preferred Skills Knowledge of CMS regulations and MDS documentation. Experience with electronic medical records. Strong organizational and time-management abilities.
