Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care. Applies the nursing process based on theory, research, evidence-based practice, and approved organizational nursing standards. Assesses and evaluates patient care needs while applying critical thinking skills in patient care management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care. Prioritizes all aspects of patient care, including teaching, rounding, coaching, and planning after hospital care, delegating to others as appropriate. Educates patients and caregivers about their plan of care, transition of care, promotion of health, and prevention of disease, anticipating needs and readiness to learn. Communicates effectively and professionally with patients, families, and all members of the interdisciplinary patient care team. Manages assignments within the Care Delivery Model, demonstrating effective allocation of material resources. Role models the tenets of Relationship Centered Care through various patient care actions. Considers the patient’s values, preferences, cultural diversity, expressed needs, and knowledge in all aspects of care. Establishes and maintains therapeutic relationships with patients and their families. Uses evidence-based practices to enhance understanding of patient perceptions of care. Initiates service recovery for patients and family members who have concerns, escalating as needed. Demonstrates caring, respect, compassion, empathy, and active listening in all interactions. Supports staff in using evidence-based practices to enhance understanding of patient perceptions. Conducts Purposeful Hourly Rounding on assigned patients to address their needs. Practices safe and efficient patient-centered care, identifying and escalating barriers for improvement. Demonstrates clinical skills and knowledge critical to quality and safety in the patient population served. Implements process improvement strategies based on established procedures. Engages in peer and institutional review processes to support quality and safety. Collaborates with interdisciplinary teams on quality improvement projects and nursing research studies. Contributes to quality and safety practices and compliance measures. Implements high reliability principles to create a safe environment for patients. Escalates safety hazards or gaps from best practices following the chain of command. Documents nursing practice to support quality and performance improvement initiatives. Performs charge nurse duties as assigned. Coordinates with clinical coordinators and managers to ensure supplies are used appropriately. Escalates resource deficits to the appropriate management. Demonstrates patient-focused and cost-effective approaches to patient care. Documents accurately and thoroughly in compliance with legal and regulatory standards. Facilitates patient throughput effectively on shift. Contributes to the environment of care supporting clinical colleagues, patients, and families. Advances clinical competence in nursing practice. Projects a professional image and communicates with respect and empathy. Incorporates ethical principles into decision-making for patient and family care.
