Care Coordinator

Grand Peaks is looking to hire a full time Care Coordinator to join our Idaho Falls Medical team. This role is a good fit for someone who enjoys working with patients, building relationships, and helping people navigate the healthcare system. You’ll help coordinate referrals, follow-up care, and services while helping patients understand their care and overcome barriers along the way. Full-Time | Monday–Friday | 8:30 AM–4:30 PM | No Weekends | Pay: DOE Job Duties: Coordinate care for patients with chronic conditions and complex or high risk healthcare needs. Serve as a point of contact for patients and families, helping them navigate healthcare services and address questions or concerns. Coordinate referrals, appointments, medical records, and communication between primary care providers, specialists, hospitals, and community resources. Follow up with patients after emergency department visits and hospitalizations to support timely follow up care. Help patients understand their care plans, medications, referrals, and health goals. Identify barriers to care and connect patients with resources such as transportation, prescription assistance, interpreters, financial support, and community services. Communicate with patients by phone and in person to provide education, answer questions, and monitor progress. Maintain accurate documentation in the electronic medical record. Work closely with providers and the healthcare team to support coordinated, high quality care. Requirements 2+ years of experience working with patients with chronic conditions, families, or other high-need populations preferred. Previous care coordination or care management experience in a medical setting is preferred. Strong communication, organization, problem-solving, and follow-through skills. Ability to manage multiple priorities and work independently. Knowledge of medical terminology and electronic health records. A compassionate, respectful, and patient-centered approach to care.
