Wound Care Nurse (RN or LPN) Description Department: Kresge Nursing & Rehabilitation Employment Type: Full Time Location: Chelsea, MI Schedule: Exempt, min 40 Hours per week | Monday-Friday | 7 team on call rotation | No Holidays Department: Chelsea Retirement Community | Kresge Healthcare & Rehabilitation Center | Chelsea, MI What You’ll Do: Make a meaningful impact by helping residents maintain skin integrity, heal effectively, and prevent complications before they start. As our Wound Care Nurse , you’ll take the lead on the facility’s wound care and skin prevention program, combining hands‑on clinical care with assessment, care planning, education, and interdisciplinary collaboration. In this role, you’ll complete weekly wound and skin assessments, coordinate treatment updates with providers, follow new admission skin concerns, maintain wound reporting, and help ensure preventative measures are in place for residents at risk. You’ll also serve as a clinical resource for the team by providing education on pressure injury prevention and supporting consistent, evidence-informed wound care practices. Key Responsibilities Complete weekly wound and skin assessments, including measurements, documentation, and treatment follow‑up. Coordinate wound care treatments and treatment changes with providers and the interdisciplinary team. Review new admission skin assessments and ensure appropriate interventions are initiated. Develop and update care plans related to wound treatment, skin integrity, and prevention. Maintain wound tracking and reporting, including participation in Quality of Life/Risk meetings. Provide education and support to team members on pressure injury prevention and preventative skin care. Skills, Knowledge and Expertise Required Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the State of Michigan. Current CPR or BLS certification . Previous experience working in a skilled nursing or long‑term care setting . Ability to lift and move up to 35 pounds without assistance . Preferred Knowledge of and experience with Skilled Nursing Facility clinical operations or another appropriate healthcare setting. Previous wound care experience . Wound Care certification preferred.
