This remote telehealth opportunity focuses on Medication Therapy Management (MTM) consulting and offers a chance to support patients across multiple states. The assignment spans 13 weeks, with potential extensions, and is designed for a pharmacist with multiple state licenses, enhancing the prospect for ongoing full-time work. Louisiana serves as the central hub, providing particularly steady work for those licensed there until the MTM program expands nationwide.

Key Responsibilities:

• Conduct virtual patient consultations through telephonic or video Comprehensive Medication Reviews (CMRs) and Targeted Medication Reviews (TMRs), addressing side effects, therapy questions, and adherence improvement.
• Evaluate utilization management tasks including prior authorization requests and formulary exceptions based on clinical guidelines.
• Collaborate with prescribers via secure messaging or phone to suggest cost-effective and formulary-compliant alternatives when necessary.
• Perform proactive clinical outreach to identify and address care gaps for chronic diseases such as diabetes and cardiovascular conditions.
• Support formulary processes by translating clinical intent into claim adjudication rules and reviewing complex claims data.
• Maintain compliance with HIPAA, telepharmacy regulations, and client contracts, ensuring thorough documentation of all clinical activities.

Qualifications & Experience:

• Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy degree.
• Active, unrestricted pharmacist license in one or more states; multi-state licensure strongly preferred.
• Minimum 2 years clinical pharmacy experience, ideally in pharmacy benefit management, managed care, or telehealth environments.
• Sound clinical knowledge in chronic disease management, drug utilization reviews, and formulary navigation.
• Comfortable with telehealth platforms, electronic health records (EHR), and PBM claims systems.
• Excellent verbal and written communication skills for patient interactions and professional consultation with providers.

Locations:This role is fully remote, supporting patients primarily in Louisiana initially, with planned expansion to 30-40 states by the fall. States of interest include Alaska, California, Maryland, Massachusetts, Virginia, and others.

This contract spans 13 weeks with onboarding surges multiple times annually, plus possible extensions and the option for direct hire positions at the onsite hub.
