SHARP (small high acuity/risk panel) is a Nurse Practitioner led small panel primary care program. Johns Hopkins SHARP providers co‑manage a small panel of high‑risk adult patients in a primary care practice, enhancing standard PCP care. The SHARP NP collaborates with 3‑5 established PCPs to provide enhanced, high‑touch, independent and collaborative primary care to a select group of high‑risk patients. SHARP NP enjoy extended visits and significantly lower visit volumes than seen in standard primary care practices.
