Responsibilities
- Conduct initial and ongoing physical therapy evaluations for residents in a geriatric care setting
- Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence
- Provide evidence-based interventions (gait training, therapeutic exercises, and modalities as appropriate) and monitor progress
- Reassess progress, adjust treatment plans, and participate in discharge planning with the interdisciplinary team
- Document evaluations, progress notes, treatment plans, and outcomes in the resident’s medical record
- Ensure safety during therapy sessions and adherence to infection control and safety policies

Requirements
- 13-week travel assignment availability
- Experience in geriatric or skilled nursing rehabilitation preferred
- Strong clinical documentation and communication skills
- Ability to collaborate effectively with an interdisciplinary care team
- Comfortable working the specified shift pattern and assignment duration
- Willingness to work on-site in a skilled nursing care setting
