RN Clinic Care Coordinator

Location: Jacksonville, FL FTE: Full-Time (1.0 FTE) Shift: Days - 7:30AM - 4:30PM

The RN Clinic Care Coordinator directs, coordinates, and monitors clinical activities within an ambulatory clinic setting. This individual will interface with the multidisciplinary health care team to ensure care coordination and delivery of needed care services. This individual closely collaborates with department physicians and clinic leadership to ensure care coordination actions are consistent with the goals of patient care and the practice. Assist patients and caregivers in navigating appointment referrals and follow up issues.

Key Responsibilities

• Directs and coordinates clinical activities within the ambulatory clinic.
• Collaborates with physicians, clinic leadership, and multidisciplinary healthcare teams.
• Coordinates the delivery of necessary patient care services.
• Ensures care coordination activities align with patient care and practice goals.
• Assists patients and caregivers with navigating appointments and referrals.
• Coordinates follow-up care and helps address issues or barriers to accessing services.
• Promotes continuity of care and a positive patient experience.

Minimum Qualifications

Education

• Bachelor's Degree required
• BSN from an accredited school of nursing

Licensure

• Current Registered Nurse (RN) license in the State of Florida

Experience

• 12 years of related clinical experience

Skills

• Strong team-building skills
• Effective verbal and written communication skills
• Experience with conflict resolution and negotiation strategies
• Proficiency with Microsoft Excel, Word, and PowerPoint

Preferred Certification

• Ambulatory Care Nursing Certification, or
• Care Coordination and Transition Management Certification (AAACN)
