Current, unencumbered license to practice physical therapy in the state of assignment. 2-3 years of physical therapy experience preferred. Current CPR/BLS certification from AHA or ARC. Experience delivering skilled physical therapy in home health or community settings, including patient evaluations, plan‑of‑care implementation, and discharge planning. Ability to supervise and evaluate Physical Therapy Assistants and Home Health Aides as assigned. Familiarity with documentation systems and basic computer skills for electronic health records, e‑mail, and online training platforms. Reliable transportation, valid driver’s license, and required agency liability coverage for home visits. Assignment Details Facility Type: Home Health. Shift Name: Days. Shift Pattern: 5×8.
