Care Team ManagerTidewell Hospice, a part of Empath Health is seeking a Care Team Manager to join our team. The Care Team Manager is an integral component of the leadership team with responsibility for conveying the Mission/Vision to the community served. The Care Team Manager is directly responsible for compliance with governing state and federal regulations as well as standards of care guideline requirements of accreditation agencies. Through oversight and supervision of clinical and non-clinical staff, the Care Team Manager has the primary responsibility and authority for maintaining quality of patient care in accordance with the organization's objectives and policies through planning, coordinating, implementing, and evaluating the services provided to patients and families.Since 1980, Tidewell Hospice has proudly served families across southwest Florida with compassionate, dignified care. As a member of Empath Health, we help patients with advanced illness live meaningfully while supporting their families with warmth, expertise, and respect.What You'll DoPromotes and models the organization's vision, mission, and values, making decisions and implements strategies that are aligned with the organization's business goals and objectives, industry trends, and external forces.Develops and executes goals and strategic plans for own area of responsibility and the broader organization to achieve exceptional results.Leads/manages a diverse clinical and non-clinical staff (nurses, home health aides, etc.). Attracts, develops, and retains a diverse, high performing team. This includes assessing performance of each staff member and coaching/counseling appropriately, as well as completing formal performance evaluations.Oversees staff and makes sound decisions with the care of our patients/clients in mind. This includes developing and implementing best practices to enhance the patient/client experience.Seek and respond to feedback from internal/external customers to improve overall service and patient care. Demonstrates advancement in knowledge and expertiseCertifications: Valid CPR certification from the American Red Cross or American Heart Association; Current certifications as applicable to their professional disciplineMust have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance, a copy of which will be placed in the employee's HR file.Physical Requirements: Push/pull/lift 50 lbs.
