Speech Language Pathologist (SLP)

Position: Speech Language Pathologist (SLP) providing direct speech therapy care to patients in a skilled nursing facility setting. Responsibilities: Assess, establish, and modify functional treatment goals for patients. Implement and follow speech therapy treatment plans according to physician referrals and regulatory standards. Chart patient progress accurately and timely using therapy software. Collaborate with interdisciplinary teams, patients, and families to ensure comprehensive care. Oversee and evaluate care provided by Speech-Language Pathology Clinical Fellows (CFYs), if applicable. Participate in community health initiatives and maintain professional relationships. Adhere to confidentiality, safety, and organizational policies. Maintain licensure, certifications, and continuing education requirements. Qualifications & Skills: Graduate of an accredited speech therapy program (MS or MA in Speech-Language Pathology). ASHA certification (CCC) or eligibility for Clinical Fellowship Year (CFY). Current state licensure and CPR certification. Strong communication, decision-making, and teamwork skills. Proficiency in Microsoft Office and therapy software. Ability to perform patient assessments, implement treatment plans, and demonstrate professionalism. New graduates are encouraged to apply; one year of experience preferred. Key Duties: Provide patient-centered speech therapy services for diverse patient populations with orthopedic, neurological, cardiopulmonary, and post-acute needs. Support patient recovery, functional progress, and quality of life within a supportive environment. Participate in compliance, safety, and quality assurance activities.
