As a RN Navigator, you will be joining a team of bright individuals who pride themselves on maintaining a collaborative, positive team environment. You will work with orthopedic surgeons and with patients at both Banner Del Webb and Boswell. The incumbent in this position works with patients from the time they are scheduled through post‑op consultation. You will educate and help guide patients through their journey in receiving orthopedic surgery with Banner Health. This is a full‑time day shift position working 4'10s with flexible hours. It has 391 licensed beds and provides a wide range of services including acute medical and surgical services, intensive care, emergency and urgent care, inpatient/outpatient surgery, cardiac catheterization, neurology, orthopedics, oncology, urology, pulmonary, obstetrics and gynecology, outpatient diagnostic services and adult behavioral services. Position Summary This position provides comprehensive care coordination, supporting a holistic and coordinated approach across the continuum of care. It assists with informed decision‑making; collaborating with a multi‑disciplinary team to allow for timely screening, diagnosis, treatment and increased supportive care throughout the patient experience. The position provides individual assistance to patients, families and caregivers to help identify and overcome barriers which may hinder quality medical and psychosocial patient care. Core Functions Assesses the patient’s plan of care and develops, implements, monitors and documents utilization of resources and progress of the patient through care, facilitating options and services to meet the patient’s health care needs. Evaluates the medical necessity and appropriateness of care, optimizing patient outcomes; serves as a liaison for referring physicians and assists with scheduling initial tests and consultations. Assists the patient/family through diagnostic services, treatment and care, coordinating disease care using an interdisciplinary holistic approach, making appropriate referrals and consultations in coordination with physicians and providers. Ensures that the patient and family understand the diagnostic processes, care strategy and recommended actions, and are provided with appropriate information in coordination with physicians and health care providers. Responds to patient requests for information regarding the disease process, expected side effects and community resources. Evaluates patient’s functional abilities and limitations, determines if intervention is needed, establishes treatment goals that are functional, measurable, patient‑related and reflect key limiting factors, and implements a plan of care to achieve those goals. Collaborates with patient and family when setting goals, initiates discharge planning, and recommends additions or modifications of referring orders. Supports the patient and family during difficult decision‑making periods, and assists in coordination of end‑of‑life care and provides emotional support. Supports process improvement activities for populations of patients to achieve optimal clinical, financial, operational and satisfaction outcomes. Participates in staff development to maintain current standards of practice and ensure the highest quality of care. Establishes and promotes a collaborative relationship with physicians, payers and other members of the health care team, collecting and communicating pertinent, timely information to payers and others to fulfill utilization and regulatory requirements. Works independently with freedom to determine how best to accomplish functions within established procedures. Conforms with supervisor on any unusual situations. Works facility‑based with no budgetary responsibility. Mandated internal and external customers described. Requires complex problem‑solving related to plan of care, evaluation, treatment and discharge planning. Minimum Qualifications Graduate with a Bachelor’s degree in nursing or related field. Current RN license in the state of practice and BLS certification. Approximately five years of clinical experience and proficiency in specialty care services. Excellent organizational, clinical knowledge, communication, writing, time‑management and proficiency with word‑processing and database software. Ability to teach clinical and non‑clinical personnel regarding care and diagnostics services, and a good understanding of process improvement.
