The Complex Case Expert will work directly with clients involved in AOT Enhanced Voluntary Agreements. This role serves as a primary point of contact to explore opportunities to divert individuals from being placed on an AOT court order, facilitate step-down from an existing order, or provide transitional support following a recent psychiatric hospitalization, ensuring connection to appropriate care and services. This role will be driven by collaboration with the individual through a person centered and recovery oriented framework.CCSI is partnering with Monroe County Office of Mental Health (MCOMH) to hire a Complex Case Expert, AOT Enhanced Voluntary Agreements. This position is employed by CCSI and working on behalf of Monroe County Office of Mental Health.In this role, you willWork with existing AOT staff to implement the county AOT Plan including EVAs and other required AOT duties.Coordinate care with AOT/EVA service providers, local hospitals, OMH and all other AOT/EVA stakeholders.Ensure collaborative agreements are in place with hospitals in support of improved discharge planning for individuals being considered for AOT or EVAs.Provide technical assistance to providers supporting individuals on EVAs.Monitor county utilization of EVA vs. AOT; routine review of data, trendsWork with local law enforcement to ensure appropriate use of removals and coordination of crisis response.Interact with and participate in discharge, family, and treatment team meetings (with hospitals, outpatient, family, jail, etc.) to reinforce EVA involvement.Review referrals for EVA determination.When needed connect new referrals to care management and/or clinical agencies upon completion of an EVA.Review and record progress notes in a comprehensive, timely manner.Provide continued care coordination for individuals on or stepping down from EVAsCommunicate in a professional manner as a representative of Monroe County to all providers and partners, including to local County, City, and Justice Courts.Travel as necessary to fulfill responsibilities.Maintain a flexible schedule to meet the needs of the programPerforms other duties as assignedWhat you bringBachelor’s degree in human services field and minimum of 5+ years of work experience or equivalent education and work experience (see CCSI's equivalency chart)Additional professional experience with the justice system, criminal procedure law, mental hygiene law are preferred, and Assisted Outpatient Treatment preferredRequired for this position are:Respect the confidential nature of all CCSI and CCSI customer information you are exposed to in the course of your work performance.
