Registered Nurse Care Manager, 4E, 40 Hours (Four 10hr Days / Rotating Weekends)

Position Summary Utilizing a collaborative process, the care manager will assess, plan, implement, monitor, and evaluate the options and services required to meet an individual’s health needs, using communication and available resources to promote quality, cost‑effective outcomes. The care manager helps identify appropriate providers and facilities throughout the continuum of services while ensuring that available resources are used in a timely and cost‑effective manner to obtain optimum value for both the patient and the reimbursement source. This role is dedicated to handling increased volume and supporting the overall care management process and the department. Job Requirements Education Graduate of an accredited BS program in Nursing preferred. Nursing degree: Diploma, ASN or BSN (preferred), with ability to obtain a BSN within 4 years. Licenses & Certifications Licensed to practice professional nursing as a Registered Nurse in the Commonwealth of Massachusetts. Experience A minimum of 3‑5 years of recent clinical experience required, and 1‑3 years of recent acute, inpatient care management experience required. Knowledge & Skills Understand confidentiality and the legal and ethical issues pertaining to patient health. Proficiency in medical terminology, accurate history taking, establishing treatment goals, and forming working relationships with referral sources. Assess an individual’s level of physical/mental impairment and understand the physical and psychological characteristics of illness. Assist individuals in developing short‑ and long‑term health goals. Manage prior approval requirements by payers and evaluate the quality of required medical services. Acquire and analyze cost of care, and understand various health care delivery systems and payer plan contracts. Demonstrate cost savings and apply case management principles and problem‑solving techniques. Document care management services, understand liability issues, and access and evaluate resources to meet client needs. Exhibit excellent interpersonal, verbal, and written communication and negotiation skills. Strong analytical, data management, and PC skills, with current working knowledge of discharge planning, utilization management, case management, performance improvement, and managed care reimbursement. Understand pre‑acute and post‑acute venues of care and post‑acute community resources. Strong organizational and time‑management skills, with the capacity to prioritize multiple tasks. Ability to work independently and exercise sound judgment in interactions with physicians, payers, and patients and their families. Compensation & Benefits Compensation Range: $38.05 – $75.33 per hour.
