Location: Montana (Poplar or Wolf Point – final site TBD)
Position Type: Full-Time | Contract
Duration: 1 Year
Start Date: As soon as security cleared
A long-term contract opportunity is available for a Nurse Case Manager to support patient care coordination services in Montana.
Position Highlights
Coordination of patient care and services
Management of referrals and external services
Collaboration with providers and community resources
Stable, long-term contract role
Qualifications
Active, unrestricted RN license
Purchased/Referred Care (PRC) experience required
Prior case management experience preferred
Ability to obtain required security clearance
