Job Title: RN Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 12 months Shift: Monday - Friday Hours per Shift: 8:00 AM - 4:30 PM Experience: Minimum of 2 years of recent RN Case Management experience License: Current RN license required Certifications: BLS certification required Must-Have: - Experience in a Long-Term Acute Care (LTAC) or Short-Term Acute Care Client (STACH) setting - Familiarity with MCG guidelines - Experience in discharge planning across acute care settings - Strong interdisciplinary team collaboration skills Description: Coordinates management of care and ensures optimum utilization of resources. Ensures service delivery and compliance with external review agencies. Provides ongoing support and expertise through comprehensive assessment. Develops care plans and implements strategies based on individual patient needs. Evaluates overall patient needs to enhance quality of management and satisfaction. Promotes continuity of care and cost-effectiveness through integration of functions. Engages in case management, utilization review, management, and discharge planning. Serves as a patient advocate by addressing holistic needs during resource utilization. Supports effective discharge planning through patient-centered approaches.
