Key Qualifications:

• Licensed Occupational Therapist (OT) in good standing
• Experience working in hospital, acute care, and/or long-term care settings preferred
• Strong assessment and clinical documentation skills

Responsibilities:

• Evaluate, develop, and implement individualized treatment plans for patients in acute care and the long-term care facility
• Deliver therapy interventions and adapt activities to maximize patient independence
• Effectively monitor progress, adjusting treatment as needed based on patient response
• Document treatment sessions and communicate progress with healthcare team and family members
• Participate in care team meetings and contribute to discharge planning
