Job Summary As a Hospice Education Specialist, you will help expand our reach and connect more patients and families with the care they deserve by building relationships with healthcare referral sources, educating the community, and representing Magnolia Hospice with professionalism and compassion. Essential Functions Referral Source Development Build and maintain relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, and other referral partners. Conduct regular visits and calls to referral sources to promote awareness of home health services. Community Engagement Represent Magnolia Hospice at health fairs, community events, and networking opportunities. Deliver presentations to healthcare professionals, community leaders, and organizations to build awareness and understanding of the benefits of timely hospice care. Education & Advocacy Communicate eligibility criteria, home health philosophy, and Magnolia Hospice’s personalized approach to care. Advocate for patient and family needs by ensuring access to services when appropriate. Collaboration with Clinical Teams Work alongside intake and clinical teams to ensure smooth transitions from referral to admission. Provide market feedback to leadership to support service quality and growth strategies. Performance Accountability Meet or exceed monthly referral and admissions goals. Document all contacts, meetings, and activities in the company CRM in compliance with HIPAA and policy guidelines. Additional Responsibilities Performs other related duties as assigned or requested. Conforms to all applicable Agency policies and procedures. Participates actively in continuing education and in‑services.
