The role focuses on coaching individuals with Type 2 Diabetes via phone and chat, coordinating with dietitians and nurses, and guiding sustainable health behavior change. Responsibilities include delivering 15–30 minute sessions, applying motivational interviewing, and using Time in Range and Stage of Change to tailor plans.
