Vice President OF Clinical Services

Overview The Vice President (VP) of Care Management provides strategic and operational leadership for care management services across the Mount Sinai Health System (MSHS), including Case Management, Social Work, and Care Transitions. Reporting to the Deputy Chief Clinical Officer (DCCO), the VP is accountable for optimizing care coordination, resource utilization, discharge planning, and patient throughput across all sites of care. This leader works in close collaboration with clinical and operational leaders to implement system-wide strategies that enhance quality, reduce length of stay (LOS), and support value-based care delivery. Responsibilities Strategic Leadership & System Integration Design and lead a system-wide care management strategy aligned with MSHS clinical transformation priorities. Collaborate across departments to align care coordination with organizational goals, especially LOS optimization and discharge efficiency. Represent care management in cross-functional forums, including throughput committees and transformation workstreams. Operational Oversight & Standardization Oversee Case Management, Social Work, and Care Transitions teams across all MSHS sites. Ensure consistency in workflows, documentation, and discharge planning practices. Support appropriate level of care decisions (e.g., admission, observation, home-based care). Analytics, Technology, and Learning Health System Partner with the Clinical Command Center and Digital & Technology Partners to utilize data for real-time decision-making. Monitor and analyze key metrics related to LOS, denials, and barriers to discharge. Support the development and use of advanced tools for throughput improvement and post-acute planning. Team Leadership and Development Lead, mentor, and develop a high-performing care management leadership team. Promote professional development and succession planning across the care management structure. Foster a collaborative, inclusive, and accountable culture. Financial and Resource Stewardship Collaborate with finance leaders to align care management operations with system cost-containment strategies. Oversee budget development and identify areas for efficiency and value capture. Support appropriate utilization management to minimize denials and enhance financial performance. Regulatory Compliance and Audit Readiness Ensure compliance with CMS, Joint Commission, and all relevant regulatory requirements. Oversee documentation practices to support audit readiness across all domains. Lead preparation for internal and external quality and compliance reviews. Stakeholder Engagement Serve as liaison between care management and nursing, medical, and community stakeholders. Represent the health system in external partnerships related to care transitions and post-acute care. Promote interdisciplinary collaboration across sites and service lines. Qualifications Master's degree in Nursing, Social Work, Healthcare Administration, or related field. Active RN license or other relevant clinical licensure required. Certification in Case Management (e.g., ACM, CCM) preferred. Minimum of 10 years of progressive leadership experience in care management or clinical operations, with 5+ years at the senior leadership level. Demonstrated success in LOS management, discharge planning, and utilization optimization across a large, multi-site health system. Deep understanding of CMS, Joint Commission, and other relevant regulatory standards. Strong skills in financial stewardship, performance management, and strategic planning. Experience leading in an academic medical center or integrated delivery network preferred. Familiarity with Epic or comparable EHR and clinical analytics tools. Ability to lead teams and drive initiatives independently with minimal oversight. Strong analytical and decision-making skills, especially in complex, matrixed environments. Ability to interpret clinical, operational, and financial data to support strategy execution. Capability to engage cross-functional teams and foster interdepartmental collaboration. Strong written and verbal communication skills for internal and external stakeholder engagement. Working Conditions Travel across MSHS hospitals and ambulatory sites is expected.
