Our client has a permanent (direct hire) position located in the greater Jacksonville area for a Physical Therapist. This is an established Home Health/Therapy, company that has a high rating and great track record.

This is a licensed, Medicare-certified Agency dedicated to providing exceptional patient-centered care in the comfort and safety of our clients’ homes. Their mission is to support health, independence, and dignity through skilled nursing, therapy, and support services.

This position can be either full-time or part-time. You have the ability to choose what works for you.

Very lucrative compensation per visit, mileage reimbursement, PTO, CE reimbursement, and healthcare reimbursement. There is the ability to make more than a clinic setting!

Compensation : Physical therapists will be mainly performing SOC, Evaluations, etc. The PTA’s on staff handle a majority of the routine visits.

• SOC ( start of care ) $115/ per admission
• PT Eval/ Reeval/ DC/ ROC ( resumption of care )/ RC ( recertification) $85
• Routine visit $65
