Lyra Caregiver Assistance Program - Up to 25 counseling or coaching sessions per eligible member per year The Physical Therapist manages and provides skilled physical therapy to patients/clients with a wide variety of diagnoses and disabilities, taking into account the individual patient special physical or age-related needs. Requires skills in patient/client evaluation and treatment, as well as professional consultations to achieve maximum patient/client independence in accordance with the individual functional level and potential. • Bachelor's Degree From an accredited Physical Therapy Program.
• Master's Degree From an accredited Physical Therapy Program.
• D.P.T. From an accredited Physical Therapy Program.
• Coursework/Training: Occasionally a position will require specialized

physical therapy training to meet a particular department need.

• Upon hire: Oregon Physical Therapist License Or
• Upon hire: Washington Physical Therapist License
• Upon hire: National Provider BLS - American Heart Association
• Occasionally a position will require specialized physical therapy experience
