RN Case Manager — HIV Prevention & Care Coordination

Inova Health System’s Juniper Clinic is seeking a dedicated Registered Nurse Case Manager to join the Case Management Team in Northern Virginia. This is a full-time day-shift role on-site with standard 8:00am–4:30pm or 8:30am–5:00pm hours. The RN Case Manager will coordinate HIV prevention services, provide education, and navigate care pathways for patients while collaborating with an interdisciplinary team to ensure culturally competent, patient-centered care. #J-18808-Ljbffr Inova Health System
