Community Health Navigator - Connector I

CHRISTUS Health is looking for a dedicated AmeriCorps Community Connector I in New Braunfels, Texas. This role involves providing culturally competent screening and navigation services to connect individuals with social and medical resources. Candidates will collaborate with a team to develop care plans and support chronic disease management.
