Provider Type: Physician Coverage Type: Scheduled Clinical Hours EMR System: EPIC Practice Setting: Outpatient Patients Per Shift: 16 Does your Facility require physicians to be board certified?: Yes Fellowship: NA Minimum Board Certification: Certified Estimated Credentialing Timeframe (Days): 90 What is the license requirement to bid?: Licensed in CA only Includes some Chronic Pain Management Chart closure within 72 hours Schedule = 18 PPD, minimum see 18 PPD Monday-Friday 8-5 A mix of peds/adults (Will consider IM only) A decision is typically made within 7-14 days We will also accept DO BE will be considered if truly BE (5 years or less since residency) Active DEA needed before the start Credentialing takes 30-90 days (six weeks is common if the provider submit app./supporting documents timely) The following immunizations are required for credentialing (before start date): 2022-2023 Flu Vaccine, TB Status/X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years).
