What is the license requirement to bid?: Licensed in CA only

Estimated Credentialing Timeframe (Days): 45-60

Mon-Friday 8-5/ Tuesday- Friday 7a-6p

50/50 kids/adults

The provider must be willing to work 3 months minimum with the option to extend. No exceptions.

Chart closure within 72 hours

A decision is typically made within 7-14 days

Active DEA needed before start

Credentialing takes 30-60 days (six weeks is standard for Locums who submit app./documents timely)

The following immunizations are required for credentialing (before start date): 2022-2023 Flu Vaccine, TB

Status/X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete

Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer

or Complete Immunization Series, Tdap Immunization (must be within the last 10 years).

HCO&Vendor agree to a 30-day notice for ending all assignments.
