Position Summary

To direct and provide social work services, including case assistance, case coordination, case management at the Helen Harris Residence, a Section 2020 with 108 units for elderly and disabled, for diverse seniors in the immediate neighborhood.  Coordination with Staff of Two Bridges regarding care coordination. Determine client's needs and service preferences through service assessments; working with each client and family members to develop an individualized care plan; identifying programs, services, and resources to meet care plan goals; and monitoring the care plan by maintaining contact with client/family and service providers, adapting the care plan as client needs change.

Responsibilities

Case Management

Provide the following case management services in Chinese and English languages:

• Information and referral  
• Advocacy - Supportive Contact
• Benefit and entitlement assistance 
• Develop and maintain care plans                
• Financial planning
• Recognize assess and refer for mental health issues, depression, anxiety, bereavement, psychosomatic illnesses, family issues, maladaptive behaviors, cross-cultural barriers/stress, and dementia.

Service Coordination

• Provide outreach
• Provide home visits
• Coordinate services with health professionals as needed
• Coordinate care and services clients receive from various auspices: home health care, benefit & entitlements agencies, legal and financial service providers, etc.
• Provide annual housing recertifications

Administration

• Maintain compliance with HUD guidelines with HUD guidelines
• Participate in trainings 
• Compile and submit reports and vouchers on regular basis
• Work with management company and building staff (manager, super., security guard, etc.) on necessary issues
• Participate in regular administrative staff and senior service coordination meetings

Other Duties

• May assist with senior service wide events.
• Participate in regular supervision with Assistant Executive Director.
• Other appropriate duties as assigned

Qualifications

• Bachelor's degree in a Human Services field required; Master's degree preferred in social work, counseling, psychology, gerontology, or a closely related discipline
• Bilingual proficiency in English and Chinese required
• Three years of experience working with older adults preferred
• Sensitivity to and understanding of the needs of older adults 
• Ability to handle complex and difficult situations in a professional manner. Demonstrated ability to conduct one's self in a calm and professional demeanor when dealing with the public and/or with difficult situations
• Ability to develop collaborative relationships with designated community groups 
• Excellent computer skills required including experience with data management systems, willingness to learn other programs as required.   

• Strong verbal and written Communication skills
• Highly organized and detail oriented; able to plan, manage and successfully complete multiple concurrent projects, frequently with short deadlines   
• Ability to work independently
• Strong commitment to mission of HMH

Compensation: $36.80 per hour

Hours :                            Monday to Friday, 9:00AM to 5:00PM, 35 hours per week
