Music TherapistAre you looking for a workplace where you can make a genuine difference?Company Culture that feels supportive, genuine and appreciative of all?Anchor Health is committed to the communities of which we serve, the patients and families we have the honor of caring for and the EMPLOYEES who have chosen us as their work family.The Music Therapist provides an initial assessment for referred hospice patients, determines therapeutic goals, and works with individuals, family members, and staff to meet these goals using a systematic application of music interventions in the patient's homes or alternate community settings.Responsibilities:Assess and provide individualized music therapy consultants and services to the patient and/or familyDevelop individualized music therapy plan of careUses creative therapeutic activities for the purpose of improving or maintaining mental and physical function and quality of lifeImplement the music therapy interventions in accordance with the interdisciplinary plan of careDirect and implement legacy projects for patients/families as identified in the plan of careDirect other care team members in the use of music to achieve individualized care plan goalsDocument all services provided to hospice patients and families in the clinical recordTerminate therapy services when appropriateSupport the business development activities of the organizationProvide in-services to hospitals, nursing homes, and other facilities as requested and determined appropriateSupport bereavement activities as needed and appropriateProvide bereavement counseling to family members as identifiedParticipate and/or facilitate Hospice memorial servicesFacilitate grief support groups as identified by the IDGServe as the counselor at IDG meetings, participates in on call rotation, and assists with sign ups and consents for newly referred patients if neededEducate the hospice staff about the role of music therapy and the use of music therapy intervention with Hospice patients and staffRequirementsBachelor's Degree in Music with a concentration in Music Therapy and must meet educational requirements to achieve certification from the Certification Board of Music Therapist. Bachelor's Degree in Music Therapy or Master's Degree in Music Therapy Preferred. Previous experience with terminally ill patients is highly desirable.
