Health Information Specialist II

This position is located at Viera Hospital, and the schedule is M-F, 7:30am-4pm.

Position Summary

To be fully engaged in providing Quality/No Harm, Customer Service and Stewardship by maintaining accurate and complete medical records for each patient treated at Health First, assuring that the confidentiality, security, and integrity of the data and information is maintained, and provide complete and timely use of data and information, without compromising security and confidentiality. Responsibilities may vary based on job role within the department.

Primary Accountabilities

Quality/No Harm

1. Demonstrates competence in the computer applications necessary to carry out work responsibilities.
2. Proficiently navigates through the computer applications in order to identify patient by name, account number, medical record number or date of service and ability to locate appropriate chart based on that information and verifying accuracy of demographic area.
3. Proficiently navigates through Microsoft Word and Excel.
4. Responsible for verifying and sending reports and/or previous records to surgery queues or physician offices for scheduled visits 98% of the time.
5. Preps all document batches for scanning according to departmental policy and procedure at least 98% of the time.
6. Scans batches/documents and writes scanned and indexed batches to the appropriate queue according to departmental policy and procedure at least 98% of the time Indexes documents according to departmental policy and procedure to the correct patient, account and document type at least 98% of the time.
7. Performs Adjust Document Index function to include correctly inserting, splitting, moving, and appending pages/documents when and where appropriate at least 98% of the time.
8. All discharged charts are reconciled in the HIM department upon discharge. Checks daily discharge log to ensure receipt of all records and follows up on records not received at least 98% of the time.
9. Ensures that all patient records are received within 24 hours of discharge, reporting to HIM supervisor or HIM manager when unable to obtain a patient record.

Stewardship

1. Takes initiative to take on additional assignments or changes in scheduling based on daily observations in a willing manner to include all HIM Technician I responsibilities.
2. Understands and supports the processing of reports and charts to department turnaround-time goals.
3. Reviews assigned work queue(s) daily and ensures processing of assignments in the queues.
4. Productivity standards are met according to departmental policy and procedure for each job function on a weekly basis at least 98% of the time.
5. Completes flags within 24 hours or sooner as appropriate.

Customer Experience

1. Provide patient information to appropriate requesters in an accurate, timely and efficient manner according to departmental policy and procedure at least 98% of the time.
2. Understands the confidentiality of patient information and proper dissemination of this information.
3. Adheres to all Health First policies and procedures in regards to protected health information. Consults with management to clarify any concerns.
4. Provides front desk coverage to process incoming requests received by phone, fax, and in-person.
5. Consistently exhibits exceptional customer service skills and professional communication both verbally and in written format.
6. Scan and index all patient requests on a daily basis before the end of shift.
7. Always displays professionalism and respect, every person, every time.

Work Experience

• Qualifications Required:
• High school graduate.
• Previous healthcare related experience preferred.
• Proficiency in Outlook, Word, and Excel programs.
• Basic knowledge of Joint Commission and HIPAA preferred.

Physical Demands

• Visual acuity to include viewing PC monitor for extended amount of time.
• Ability to enter data using and prep charts using hand/wrist dexterity.
• Ability to appropriately transport 25 pounds of medical records.
• Ability to sit, walk, or stand for extended periods of time
• Ability to bend or stoop.
• Ability to travel to all facilities/campuses across the IDN.

• Ability to read and comprehend instructions, correspondence, memos, and electronic mail.
• Basic computer literacy and ability to enter and retrieve data from electronic systems.
