Regional Homecare Outreach & Referrals Specialist

Professional Case Management is seeking a field sales professional to build relationships with hospitals and physician offices, promote in‑home nursing services, and generate referrals. You will coordinate with Discharge Planners, Social Workers, and clinical staff to educate stakeholders and drive client onboarding. If you are driven, able to travel regionally, and have healthcare sales experience, you will help grow PCM’s patient population while supporting our mission to enhance quality of #J-18808-Ljbffr Act for Health, Inc
