As a Care Manager, you will play a key role in ensuring the safety, health and well-being of clients in the home environment. You will report directly to the Lead Care Manager.

Pay: $30-34/hr

Key Responsibilities:

• Collaborate with the Care Management team and staffing personnel to create effective care and staffing strategies.
• Conduct health and home assessments for new and prospective clients.
• Build relationships with prospects over the phone and schedule assessment visits.
• Provide orientation and ongoing training for caregiver team members, including specialized instruction when needed.
• Assess client needs and create individualized care plans that uphold our core values.
• Exercise strong clinical judgment and maintain a high standard of documentation.

Qualifications:

• Current, unencumbered Missouri RN, LPN license, MSW, or BSW with related experience
• CCM or CMC preferred but not required
• 2 plus years of experience working with geriatric patients/clients
• Valid driver's license with a good driving record
• Ability to work independently while providing excellent professional judgment
• Strong assessment, documentation, and careplanning skills
• Proficiency with Microsoft Office
