Responsibilities Provide physical therapy services to the facility, promoting excellence in rehabilitation care, interdisciplinary cooperation, and quality patient care Evaluate patients in a timely manner, as required by state, payer source, or facility requirements Screen patients upon admission and/or when requested by the director Develop an appropriate plan of treatment and goals for each patient Provide treatment according to the plan developed after patient evaluation Communicate with other disciplines and interact with staff as necessary based on the treatment plan Provide routine documentation according to Medicare, facility, and other payer source guidelines Participate in family conferences when requested Submit all required documentation and treatment charges in a timely manner, in accordance with policy and procedure Qualifications Current MA State License as a Physical Therapist Ability to work as part of a dynamic and cooperative team while ensuring workload is distributed equitably across the department Capability to evaluate and assess patients and participate in consultations and teaching Ability to collaborate with residents, staff, families, and other professionals to plan individualized treatment plans and goals Ability to discuss and plan appropriate discharge plans Ability to teach and counsel patients and staff on best practices for recovery Benefits Package 6 paid holidays Medical, dental, and vision insurance Paid time off 401(k) Supportive team environment What We Offer A dynamic and supportive work environment Opportunities for professional growth and development A chance to make a real difference in the lives of our residents Flexible Full-Time, Part-Time, and Per Diem opportunities Collaborative and dedicated therapy team Competitive wages #J-18808-Ljbffr
