You will assess patient movement, develop and implement treatment plans, document progress, and provide patient education in an outpatient clinic. Responsibilities Assess patient movement and functional status. Develop individualized treatment plans and interventions. Document treatment progress and outcomes accurately. Provide patient education and discharge instructions. Collaborate with interdisciplinary team members. Qualifications Degree in Physical Therapy from an accredited program. Active Arizona PT license or eligibility (PT Compact accepted). Basic Life Support (BLS) certification. Social Security Number, Date of Birth, and one reference from the entire work history. Preferred Qualifications Previous travel experience.
