Job Functions:
The RN Case Manager participates on the Interdisciplinary team and coordinates care that ensures patients, families and caregivers receive optimal care and resources to support their journey. The RN collaborates with other hospice staff members, volunteers, physicians and community partners to effectively implement a plan of care focused on comfort and quality of life.
Qualifications:
- Licensed as a Registered Nurse in Massachusetts in good standing
- Associate Degree in Nursing; Bachelors degree in Nursing preferred
- Hospice experience preferred
- Skilled in nursing practice and have the ability to relate and treat terminally ill patients/families
- Ability to Assess, Adapt and respond to patient needs in a variety of settings
- Must be a license and insured driver in MA
