Description

• Conduct thorough evaluations of patients in both inpatient and outpatient environments, creating personalized treatment strategies
• Implement physical therapy techniques targeted towards mobility, strength, balance, and functional improvement
• Work alongside nurses, physicians, case managers, and other rehabilitation specialists to achieve the best patient outcomes
• Educate patients and their families about therapeutic objectives, discharge strategies, and home exercise plans
• Maintain detailed documentation of assessments, treatments, and progress inline with facility and regulatory standards
• Engage in interdisciplinary care team meetings and discussions to ensure seamless patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent credential from an accredited institution

Certifications

• Valid state licensure as a PT / Rehabilitation Therapist
• CPR certification is preferred
• Special certifications (e.g., neurological, geriatric) would be advantageous but are not mandatory

Skills

• Prior experience is strongly desired
• Ability to manage patients with multifaceted medical and mobility challenges
• Excellent communication and collaboration skills
• Proficient in utilizing digital charting and clinical documentation tools
